Use present simple habits (and permanent situations)

Adverbs of frequency - position is before the main verb

I never go to the theatre.

I sometimes eat toast for breakfast.

I don't usually go to school on Wednesday afternoon.

BUT! adverbs of frequency come after the verb to be

I am never late.

Once a week, twice a month etc "expression of frequency" at the end of the sentence 

"I go to the cinema once a month."
